    Appointments are not guaranteed until insurance and availability have been verified by our office staff. If your appointment cannot be approved or if additional information is needed, we will notify you.

    ***Medicaid insurance is NOT accepted.***

  • Appointment Reminder

    You will receive a reminder via email, text, or call two days before your scheduled appointment.

    Appointment Day Instructions

    For New Patients:
    Arrive 15 minutes before your appointment.
    Complete electronic new patient packet before arriving.

    For All Patients:
    Bring a picture ID and your insurance card(s).
    Be prepared to pay your co-pay and/or deductible.
    Bring a complete list of your medications.

  • No Show / Cancellation Policy

    We kindly ask that all scheduled appointments be kept. If you are unable to attend, please notify us by phone.

    24-hour advance notice is required for cancellations or rescheduling.

    A $15 no-show fee will apply to returning patients who miss an appointment without providing at least 24 hours' prior notice.

    3 missed appointments without proper notification may result in dismissal from our practice.
